Implied Consent Form:

Hello!

You are being asked to participate in a study titled "Teacher Motivation in Foreign Language Education" which is being conducted by Anastacia Sanford, Department of Language Education at the University of Georgia ((706)542-4337) under the direction of Dr. Linda Harklau, Department of Language Education at the University of Georgia ((706)542-4521), 125 Aderhold Hall, Athens, GA 30602. The research may be published. Your participation involves completing the following survey.

The reason for this study is to find out what motivates foreign language teachers to become foreign language teachers, what makes them stay in the profession, and what makes them leave it. You may not benefit directly from this research. However, since the study will hopefully find out the factors that encourage teachers to stay in the profession, your participation in this research may lead to information that could help schools to be more effective at retaining their foreign language, and possibly other area, teachers.

If you decide to take part in this study, your participation will involve completing the following survey. The survey should take 5-20 minutes to complete. You do not have to take part in this study; you can stop taking part at any time without giving any reason, and without penalty. You can skip any questions that you feel uncomfortable answering. Even after completing the survey, you can decide not to submit it and simply reset the form or close the website to withdraw. No names will be associated with the survey and you should not mention the name of your school in any of your responses. Internet communications are insecure and there is a limit to the confidentiality that can be guaranteed due to the technology itself. However, once the completed survey is received by the researcher standard confidentiality procedures will be employed. There will be no link to you, the participant. If you would rather not submit your responses via the internet, you may print the survey out, complete it by hand, and send it US mail with no return address to Anastacia Sanford, University of Georgia, 125 Aderhold Hall, Athens, GA 30602. This will assure complete anonymity.

The researcher will answer any further questions about the research, now or at a later date, and can be reached by telephone at: 706-542-4337 or by email at: stacie78@uga.edu.

Thank you for your consideration,
